What kind of insurance is accepted?
- Dr. Hallett is currently in network with Anthem/Blue Cross Blue Shield.
- Self-pay patients are also accepted.
Can I use Out of Network (OON) benefits for services?
- Yes. Upon request, I will provide a superbill to submit to your insurance company for reimbursement. Patients using OON benefits pay at the time of service. Your insurance company may reimburse you for a portion of the services provided and/or apply the money spent toward a deductible or an Out of Pocket (OOP) maximum amount.
Can I use my HSA account to pay for services?
- Yes, mental health services provided by a psychologist are considered to be an eligible medical expense.
- Cash, personal check, and all major credit cards are also accepted.
Pricing & Rates
Therapy
- Initial intake (75-90 minutes) $250
- 45-50 minute session $150
- 60 minute session $200
Testing/Psychological Evaluations
- Full Psychological Evaluation $2,200
Includes initial intake appointment, face-to-face assessment procedures, scoring, interpretation, a complete written report, and a feedback session.
- Other testing $200 per hour
If a full psychological evaluation is not warranted, billable hours will include initial intake appointment, face-to-face assessment procedures, scoring, interpretation, report writing, and a feedback session.
- Legal Evaluations or Court Appearances
Cost will be determined on a case-by-case basis by Dr. Hallett.
Cancellation/Reschedule Policy
A cancelled or missed appointment delays therapeutic progress and our work together.
Please provide at least 24 hours of notice if you need to cancel or reschedule an appointment; for Monday appointments, notification must occur by 2pm on Friday. (Multi-hour psychological testing appointments require 48 hours of notice.) With insufficient notice, other patients who might be waiting for an appointment are unable to utilize that slot. For no-show or late cancelled appointments, you will be charged the cancellation fee as specified in the office policies provided at the time of intake. (Please note that insurance companies do not provide reimbursement for late cancellations or missed appointments.)
